What do we do?
The Complex Geriatric Clinic (CGC) offers comprehensive geriatric assessments to older people aged 65 years and above, with health or ageing related problems.
The clinic is part of the Grampians Health Service of Continuing Care, which includes CDAMS, Falls and Balance Clinic, and Continence clinic.
We take a holistic whole-person approach to the care of older adults with complex medical and functional concerns.
These problems may include issues relating to frailty, malnutrition, falls, medication-related, complex social situations and cognition.
What is required and how to proceed
- Your GP or medical specialist/nurse practitioner would need to send us a referral detailing the clinical problems, and background information of your other medical problems, current medications, any investigations and current social and functional information. This will assist with the triaging service. Any patients requiring immediate assessment should be referred to the Emergency Department of the local hospital.
- Any clinical documentation from private specialists would be very useful and should be included in the referral.

What you should bring
- Please bring along your current medications, usual walking aids (walking frame or walking stick), sensory aids (reading glasses, hearing aids)
- We encourage you to bring along a family member or support person
- Any medical documentations from your private medical specialists

What do we do?

These may include issues relating to frailty, malnutrition, falls, medication-related, complex social situations and cognition.
Please note that the CGC does not replace existing specialist clinics, such as CDAMS, Falls and Balance clinic, and Continence Clinic.
Anyone who would benefit from these specialist clinics, should be directly referred to these clinics
- Cognitive, Dementia and Memory Services Clinics (CDAMS)
- Falls & Balance Clinic
- Grampians Regional Continence Service
We do not see any patients who would be better served under the mental health service, or who primarily has alcohol and/or drug related problems.
Any single organ pathology would be better reviewed by the relevant specialist service.
What is required and how to proceed
Please send a referral with the following information to better assist with the triaging process:
- Your specific clinical question for the clinic
- Current medical problems
- Current medications and allergies
- Alcohol and smoking history
- Current living situation, social support and any safety concerns
- Most recent investigations
We accept all referrals from GP and medical specialists/nurse practitioners across the Grampians Health catchment area.
Please note that the clinic is only conducted in Queen Elizabeth Centre.
Selected patients might be offered telehealth service. Home visits and residential aged care visits are at the discretion of the triaging Geriatricians depending on clinical needs.
