Post Acute Care (PAC)

The services and support offered are designed for individual needs.  This program aims to help people recover and return to a normal lifestyle, as soon as possible, and to avoid any unplanned return to hospital.

People being discharged from a public hospital (including emergency departments, acute and sub-acute services) with limited supports at home can be referred to PAC.  

They need to be assessed as requiring short-term, community-based help to assist them to recuperate in the community and to ensure a safe and timely discharge.

Referral to the program is arranged by your hospital when you are being discharged.

Where another program was providing support prior to the hospital episode, it is expected that these services will resume on discharge from hospital. 

How does the PAC Program work?

The Care Coordinator will be notified that a client may need services.

They will:

• See them in hospital before discharge and discuss their needs, or
• Call them after discharge and discuss their needs.
• Purchase the services needed through contracted providers.
• Liaise with any community service agencies who are involved with the client’s care.
• Monitor service and care needs by regular phone contact with the client, their family and other involved agencies.
• Liaise with agencies that will provide longer term services, for example local councils.
• Make referrals to other community agencies, if needed.

Referrals to this program are accepted from hospital staff or by self referral.
