If you are an older person or a family member who is needing an assessment to access a Commonwealth-funded Aged Care Service, ACAS can assist.
ACAS is the Victorian Commonwealth-funded service that assesses the care needs of frail older people and younger people with disabilities, to facilitate access to appropriate care services. This includes approvals for the Australian Government subsidised care services.

What do we do?

We meet with each patient and their caregivers and assess their physical, psychological, medical, restorative, cultural and social needs. After each assessment, a patient’s level of support and service needs are recommended.

An assessment by ACAS may be needed to access:

  • Residential Aged Care
  • Residential Respite Care
  • Home Care Packages
  • Flexible Care
  • Transition Care Program
  • Short-Term Restorative Care

Who is this service for?

People requiring any of the above services:

  • Aged over 65 years of age
  • Aged over 50 and Indigenous
  • Those with complex care needs that are no longer able to manage at home without assistance
  • Young people with a disability may be assessed in some circumstances. People with a disability may be eligible for the National Disability Insurance Scheme (NDIS).
