An education and support service for those who have experienced an Acquired Brain Injury (ABI) or have complex neurological issue.
What do we do?
This service aims to educate, support and empower people to self-manage their health condition and remain living independently within their community.
Case management is provided by a number of health professionals, to work towards each patient's rehabilitation goals.
Patient’s treatment plans may include:
- Care coordination
- Action plans
- Education
- Support
- Referrals to additional services
- Liaison with GP

How can you access this service?
A referral from a GP or medical specialist is required to access this service.
